29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: Workplace, 321 Roanoke Avenue, Newark, NJ In an acetylene repackaging facility, employees operate and inspect "Rexarc" compressors that step up pressure of acetylene to dispense into acetylene cylinders with fill material and acetone that adsorb the acetylene under pressure: a) In the "Rexarc" compressor room, where employees operate and inspect the compressors, there is a floor hole approximately 8 inches in length and width and depth, that employees can step into while working and walking around the compressors. b) In the "Rexarc" compressor room, where employees operate and inspect the compressors, there is a floor hole approximately 14 inches in length, 6 inches in width and 12 inches in depth, that employees can step into while working and walking around the compressors. Violation occurred on or about 11/06/2019

29 CFR 1910.101(b): Cylinders were not stored or handled in accordance with appropriate standards of the Compressed Gas Association Pamphlet P-1-1965, Sections 3.3.1 and 3.3.8, which is incorporated by reference as specified in CFR 1910.6: a) Workplace, 321 Roanoke Avenue, Newark, NJ In the Acetylene Filling Room, eight (8) full acetylene 100 ft3 cylinders were painted and left to dry without protective valve caps near an emergency exit and loading dock bay door used by employees. The employer did not ensure the cylinders were chained or otherwise secured from being knocked over, creating a potential projectile, fire, and/or explosion hazard. Violation observed 11-6-19

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: Compressor Room, 321 Roanoke Avenue, Newark, NJ a) In an acetylene repackaging facility, employees perform maintenance on process equipment, such as Compressors, including but not limited to replacing a leaking blowdown valve D53J on Compressor 5 Stage 3. The employer did not implement safe work practices in that the employer did not maintain written lock out tag out procedures with specific steps to follow to isolate and control chemical, mechanical and electrical residual energy prior to conducting maintenance activities. Violation occurred on or about 09/10/2019
