On June 26, 2019, located on the north side of a commercial structure, employees were exposed to a fall hazard working from a framed tubular welded framed scaffold system as there were no inspections performed by a competent person prior to the work shift to ensure employees were protected from falling by installing a guardrail system, personal fall arrest system, or end rail systems.
