29 CFR 1926.1207(c): The training did not establish employee proficiency in the duties required by this standard and did not introduce new or revised procedures, as necessary, for compliance with this standard. A) GREAT LAKES PIPELINE SERVICES on or about June 27, 2019 at the address of 33784 Walker Road, Avon Lake, OH: The employer did not ensure employee proficiency in their duties relating to the training they had received. Employees were not adequately trained in this subpart as reflected by deficiencies discovered, to include exposing employees to potential atmospheric hazards without the benefit of testing prior to operating inside the manhole and reclassification of the confined space.
