29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an OSHA Form 301 or equivalent within seven (7) calendar days of receiving information that a recordable injury or illness had occurred as required by 29 CFR 1960.8(b):  a) 10 Harmich Road, South Plainfield, NJ 07080  All work-related injuries occurring in 2019 were not recorded on the OSHA 300 log within seven (7) calendar days of the employer receiving information that the injury occurred.  Condition noted on or about 6/26/2019.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident:  a) 10 Harmich Road, South Plainfield, NJ 07080  An in-patient hospitalization involving of an employee who was hospitalized for a work-related injury was not reported to OSHA within twenty-four (24) hours.    Violation occurred on or about 6/26/2019.
