29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition:  a) North Side of the Warehouse - On June 27, 2019, the employer did not ensure that the floor was maintained in a safe condition where material handling equipment was used. The floor hole drain, and trench drain were not completely covered along its entire length and covers in place were bent.    b) North End G Aisle - On June 27, 2019, the employer did not ensure the work floor were not kept in good repair where material handling equipment was used.  c) Z aisle - On June 27, 2019, the employer did not ensure the work floor were not kept in good repair where material handling equipment was used.
