29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 27 June 2019, and at times prior thereto, four workers were performing concrete activities without the use of head protection, exposing them to the hazard of being struck by falling objects or materials.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 27 June 2019, and at times prior thereto, a Stihl TS420 Cutquik Hand-held Saw was being used to cut rebar without the use of safety glasses or face shield, exposing the workers using the saw to the hazard of flying particles and debris to the eye(s) and face.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  On or about 27 June 2019, and at times prior thereto, the spoil pile was not at least 2 feet from the edge while workers were performing work inside the excavation, exposing the workers to the hazard of being struck by falling or rolling excavated materials or other materials and equipment.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems.  On or about 27 June 2019, and at times prior thereto, four workers were performing concrete activities within an excavation without a competent person inspecting the excavation prior to entry, exposing the workers to the hazard of a cave-in.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement.  On or about 27 June 2019, and at times prior thereto, four workers performing concrete activities to include laying sections or rebar lattice, did so while stakes of rebar were vertical at ~14 inches tall without being capped, exposing the workers to the hazard of tripping and being impaled by the rebar.
