29 CFR 1910.22(c): The employer did not ensure each employee used, a safe means of access and egress to and from walking-working surfaces.  a) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the North exit door on the main lobby unsecured  to avoid employee usage leading directly to an area identified as a construction area with roof work being performed, exposing employees to being struck with falling objects.   b) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the South exit door on the main lobby unsecured  to avoid employee usage leading directly to an area identified as a construction area with roof work being performed, exposing employees to being struck with falling objects.   c) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the West exit door on the main lobby unsecured  to avoid employee usage leading directly to an area identified as a construction area with roof work being performed, exposing employees to being struck with falling objects.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  a) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the North, door identified as emergency exit doors within the facility under normal conditions, was not properly identified as Not an Exit.   b) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the South door identified as emergency exit doors within the facility under normal conditions, was not properly identified as Not an Exit.   c) On or about 6/27/2019, 1201 Calle Ceiba San Juan, PR 00926, the CSHO observed the West door identified as emergency exit doors within the facility under normal conditions, was not properly identified as Not an Exit.
