29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about June 27, 2019, and at times prior thereto, employees were exposed to falls of approximately 14 feet while using multiple ladders were even (flush) with or only up to one foot above the landing.   Note: Abatement certification is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about June 27, 2019, and at times prior thereto, employees performing roofing activities without properly using fall protection.  Workers were exposed to fall hazards of approximately 14 - 30 feet.     Note: Abatement certification AND supporting documentation are both required for this item.   A1A Roofing LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1387078, citation number 1, item number 1a and was affirmed as a final order on May 24, 2019, with respect to a workplace located at 5865 S Stockport Avenue, Meridian, ID 83642.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  (a)  On or about June 27, 2019, and at times prior thereto, employees performing roofing activities did not understand how to properly use fall protection.  Harnesses were not worn properly, including D-rings located in the lower back.  Workers were exposed to fall hazards of approximately 14 - 30 feet.    A1A Roofing LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1387078 citation number 1, item number 1b, and was affirmed as a final order on May 24, 2019, with respect to a workplace located at 5865 S Stockport Avenue, Meridian, ID, 83642.  Note: Abatement certification AND supporting documentation are both required for this item.
