29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Sunrise Drive, Coopersburg, PA, Worksite - on or about June 28, 2019, an employee was working within an engineered trench shield system, which had an installed ladder that was set vertically and supported at a landing surface approximately seven and a half feet shorter than its working length, which would not permit for rapid and safe egress in the event of an emergency.
