29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. (a) Switch Gear Pad, North Center: On or about June 26, 2019 and at times prior thereto, a floor hole on the walking and working surface measuring on each side as 28 inches, 23 inches, 16 inches, 15 inches, 10 inches and with a depth of 16 inches was not covered. This condition exposed employees to a step-in hazard. (b) Switch Gear Pad, South Center: On or about June 26, 2019 and at times prior thereto, a floor hole on the walking and working measuring on each side as 28 inches, 23 inches, 16 inches, 12 inches, 7 inches and with a depth of 16 inches was not covered. This condition exposed employees to a step-in hazard.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): R22-03-01 Line: On or about June 26, 2019, a benching system was used as a protective system in Type C soil where the excavation depth was near or at 67 inches, this condition exposed employees to engulfment and struck-by hazards in the event of trench collapse.
