29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 309 NE 102nd Terrace in Kansas City, Missouri: Employees were exposed to struck by hazards. Excavator was observed installing water pipe in a trench that was greater than 7 feet in depth thus exposed to struck by hazards from the above dirt.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):  On or about June 28, 2019 at the commercial jobsite located at 309 NE 102nd Terrace in Kansas City, Missouri- The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in trench/excavations were protected from engulfment hazards.
