29 CFR 1910.147(c)(7)(i)(B): Each affected employee was not instructed in the purpose and use of the energy control procedure: a) Multi-Family Building Products Inc. dba Direct Lumber and Door of Colorado, 6700 Franklin Street, Denver, CO 80229: On and before June 25, 2019, the employer did not ensure that each affected employee who participates in the operation of the equipment was instructed in the purpose and use of the energy control procedure on equipment such as, but not limited to, the Klaisler Finish Roller. This condition exposed employees to pinch-point and amputation hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) Multi-Family Building Products Inc. dba Direct Lumber and Door of Colorado, 6700 Franklin Street, Denver, CO 80229: On and before June 25, 2019, the employer did not adequately guard the point of operation of the Klaisler Finish Roller, model TH214-18, serial number 1755, which was used to press plates onto lumber and finish trusses of various sizes. Employees operating this roller reached under and over the safety bar during operation, placing their extremities in the danger zone during operation. In addition, the employer did not guard the first passive output roller when handling small trusses, in that employees receiving the finished trusses handled trusses as small as 22 inches by 18 inches to prevent the trusses from falling to the ground. These employees were exposed to pinch points in-between the trusses being pushed out from the press rollers and the first passive output roller. This condition resulted in pinch-point and amputation hazards, and contributed to an employee's fingertip amputation.
