29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  On or about June 28, 2019, employees used a scaffold platform that was not fully planked.  This condition exposed employees to fall  hazards greater than 10 feet from the ground.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired.   On or about June 28, 2019, employees performing plaster activities worked on a scaffold that used split and damaged planks.  This condition exposed employees to fall hazards greater than 10 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about June 28, 2019 employees performing masonry work were observed on a residential roof without any type of fall protection.  This condition exposed employees to fall hazards greater than 6 feet.    JFM Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1364519, citation number 01, item number 01 and was affirmed as a final order on 02/09/2019, with respect to a workplace located at 7755 De Alava, Vinton, TX 79821.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 28, 2019 employees who are expected to be exposed to fall hazards were not trained in the recognition of fall protection hazards and procedures to follow when working at heights over 6 feet from the ground.
