29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Location: Recycling garage area backside of the building @ 48 Cranberry Hwy. Rochester, MA. 02770. On or about 6/27/19 an employee fell 4 feet to a concrete floor while leaving the cab of a Caterpillar Front-end loader where the cab access ladder was shop made and did not conform to the manufacturers specifications.
