29 CFR 1926.451(b)(1): Platforms on all working levels of scaffolds were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights was no more than 1 inch:  (a)Laura & Roberto Construction, Inc. at 4546 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to May 22, 2019, the exposing employer did not ensure employees were protected from fall hazards when performing work associated with EIFS installation in that working levels of scaffolding that were not fully planked. This condition exposed employees to fall hazards up to 30 feet.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  (a)Laura & Roberto Construction, Inc. at 4546 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to May 22, 2019, the exposing employer did not ensure employees were protected from struck-by and fall hazards when performing work from scaffolding in that the scaffold was not erected under the supervision and direction of a competent person. This condition exposed employees to struck-by and fall hazards  up to 30 feet.

29 CFR 1926.451(g)(4): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level in that guardrail systems did not comply to the provisions of this section  (a)Laura & Roberto Construction, Inc. at 4546 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to May 22, 2019, the exposing employer did not ensure employees were protected from fall hazards when performing work associated with EIFS installation working levels of scaffolding that did not include midrails and top rails. This condition expose employees to fall hazards of up to 30 feet.
