29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) Jobsite, Alta Vista Business Park, Lot 14, Charleroi, PA: On or about June 27, 2019 an employee was on the roof of the structure cutting in furnace flues and no fall protection systems were in place such as guardrails, safety nets or personal fall arrest systems.  The fall hazard from the edge to the ground was approximately 14 feet.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    (a) Jobsite, Alta Vista Business Park, Lot 14, Charleroi, PA:  On or about June 27, 2019 an employee was exposed to fall hazards of approximately 14 feet and the employer could not provide written certification that training from the employer had been provided to the employee.
