29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: (a) Jobsite, Alta Vista Business Park, Lot 14, Charleroi, PA: On or about June 27, 2019 an unused opening in a breaker box was not closed. A 50 amp breaker had been installed to accommodate welding equipment and the opening was not closed once the breaker had been removed.
