(a) Telecommunications Tower, 328 Stafford Forge Rd., West Creek, NJ; on or about June 28, 2019, Rise & Rise Wireless Inc. employees were installing upgraded equipment at the top of a 150 foot tall monopole telecommunication tower.   Employees were using the T-bar antenna supports of the tower as their walking/working surface.  An employee was using various tie off locations as anchorage for personal fall arrest systems without the use of a proper anchor strap.  The employee was not properly supervised by a qualified person regarding appropriate anchor points during his work activity, exposing him to potential fall hazards.       NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
