On or about June 27, 2019, at the above addressed jobsite, employees performing roofing operations from a ladder jack scaffold, eighteen (18) feet seven (7) inches, above the ground, did not have safe access to a ladder while working on the ladder jack scaffold, thereby exposing the employees to falls.
