(b) Utilidor Formwork: On or about June 25, 2019 and at times prior thereto, defective wire rope sling with certificates 389587-1-2 was not removed from service. This condition exposed employees to struck-by hazards from rigging failure.

29 CFR 1926.1153(d)(2)(ii): The employer did not assess the 8-hour TWA exposure for each employee on the basis of any combination of air monitoring data or objective data sufficient to accurately characterize employee exposures to respirable crystalline silica: Utilidor, West Side: On or about June 25, 2019 and at times prior thereto, where an employee was performing a "bushing" task on concrete and visible dust was created, the employer did not assess the 8-hour TWA exposure on the basis of air monitoring or objective data. This condition exposed the employee to inhalation hazards.
