29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) Jobsite, 2021 Sunset Avenue, Johnstown, PA 15905: Observed on or about June 19, 2019, employees installing a metal roof on a two story residential dwelling were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. The slope of the roof was approximately 8 in 12 (vertical to horizontal). The employees were exposed to a fall hazard of approximately 25 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite, 2021 Sunset Avenue, Johnstown, PA 15905: Observed on or about June 19, 2019, the employer did not provide fall protection training to employee working on the roof of a two story residential dwelling. The employees were observed working on the roof of the house while not protected from falling.
