29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    On or about 7/1/2019, in the production area at 1000 Pyratich Drive, Baldwin, GA: Authorized employees performing servicing and maintenance activities such as clearing material jammed in the blade area of the HM-4545 Double Miter saw machines, were exposed to amputation and caught-in hazards.  The employer failed to provide recognition training of applicable hazardous energy sources, magnitude, and methods for isolating and controlling energy sources.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about 7/1/2019, in the production area at 1000 Pyratich Drive, Baldwin, GA: Employees performing servicing and maintenance activities such as clearing material jammed on the HM-4545 Double Miter saw machines, and other equipment were exposed to amputation and caught-in hazards, in that; procedures developed did not clearly and specifically outline scope, energy source, and techniques authorized employee had to follow to control moving and rotating saw blades.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):    On or about 7/1/2019 at 1000 Pyratich Drive, Baldwin, GA: Where employee(s) operated a powered industrial truck with defects and deficiencies such as but not limited to loose and missing backrest mounting bracket bolts, broken welds and damage backrest assembly, damage hydraulic line pulley, loose bolts on hydraulic bracket, and battery hold down bracket not secured.  The employer failed to remove the powered industrial truck from service until the repairs could be made.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    On or about 7/1/2019, in the production area at 1000 Pyratich Drive, Baldwin, GA: Authorized employees performing servicing and maintenance activities such as clearing material jammed in the blade area of the HM-4545 Double Miter saw machines, were exposed to amputation and caught-in hazards.  The last documented periodic inspections on the Urban and Joseph Saws was 8/5/2014, and the Tiger Stop equipment was 3/13/2017.    The Magnolia Windows & Doors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 1264736, citation number 1, item number 1 and was affirmed as a final order on 11/6/2017, with respect to a workplace located at1000 Pyratich Drive, Baldwin, GA.
