29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: Location: N.E. Side of the building @ 43 Church St. Woods Hole, MA. 02543. On or about 07/01/19 an employee fell 20 feet to the ground while working from a aluminum extension ladder and had not been trained.
