29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  (a)  Office Building Front Exit Door - On or about July 1, 2019, the employees used a designated exit route door that required that a bolt latch be turned so they could gain access to the outside thereby exposing them to smoke inhalation and or burn hazards.  (b)  Warehouse NW Exit Door  -  On or about July 1, 2019, the employees were required to use a designated exit route door that was locked with a latch on it that had to be slid open before gaining access to the outside thereby exposing them to smoke inhalation and or burn hazards.  (c) Warehouse Double-wide Rear Exit Door - On or about July 1, 2019, the employees used a designated exit route door that could not be opened because it was broken thereby exposing them to smoke inhalation and or burn hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.36(e)(2): The door that connects any room to an exit route did not swing out in the direction of exit travel and the room is designed to be occupied by more than 50 people or if the room is a high hazard area (i.e., contains contents that are likely to burn with extreme rapidity or explode).  (a) Warehouse SW Entry Door - On or about July 1, 2019, the employees used a designated exit route door that did not swing out in the direction of exit travel.  (b) Warehouse NW Exit Door - On or about July 1, 2019, the employees used a designated exit route door that did not swing out in the direction of exit travel.  (c)  Warehouse SE Exit Door - On or about July 1, 2019, the employees used a designated exit route door that did not swing out in the direction of exit travel.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)  Maintenance Warehouse and Doublewide - On or about July 1, 2019 employees using hazardous chemicals such as but not limited to Bike Lust, Safety-Kleen, B'Laster a skin, and Tri-Flow to perform bike maintenance and repair had not received hazard communication training. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known:  (a) Motivate - On or about July 23, 2019, entry 334 on the CY 2019 300 log had 3 days away from work but the employer only recorded the injury as 1 day away.   (b) Motivate - On or about August 30, 2019, entry 10  on the CY 2019 300 log had an x marked in column H for days away but failed to record the number of days away in column K. 29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  (a)   Motivate International Inc. - On or about July 23, 2019, entry 334 on the CY 2019 300 log involved the individual being provided prescribed pain medication and employer filled out the 301 log on December 10, 2019  (b)   Motivate International Inc. - On or about August 30, 2019, entry 10 on the CY 2019 300 log involved the individual being provided medical treatment for a repetitive motion injury to his foot and employer filled out the 301 log on December 10, 2019.  (c)   Motivate International Inc. - On or about September 3, 2019, entry 366 on the CY 2019 300 log involved the individual being provided medical treatment and being out for 20 days and employer filled out the 301 log on December 10, 2019.
