29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  365 Boston Road Springfield, MA: On or about July 2, 2019 employees were observed engaged in low slope roof construction on a roof more than 50 feet wide with a warning line system in use but were not protected from falls when outside the warning lines by either guard rails, safety nets, personal fall arrest system or a safety monitor.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  365 Boston Road Springfield, MA: On or about July 2, 2019 employees were observed engaged in low slope roof construction and the employer had not verified training by preparing a written (training) certification record including the name of the employee trained,  date(s) of training and the signature of the person who conducted the training or the signature of the owner.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
