On or about July 2, 2019, at the above addressed jobsite, an employee performing painting operations from a tubular welded frame scaffold, thirteen (13) feet four (4) inches above the ground, was exposed to fall hazards due to the lack of guardrails.

On or about July 2, 2019, at the above addressed jobsite, an employee performing residential painting operations from a Multi-Position ladder that was placed on a tubular welded scaffold plank, was exposed to falls hazards of approximately sixteen (16) feet four (4) inches above the ground.
