29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior Front (Ground Level): An employee without the use of a protective helmet was exposed to a struck by hazard, while engaged in roofing activities below roofers, on or about 7/2/19.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from physical or chemical agents:  a) Exterior Front: Employees engaged in roofing work were using hand-held pneumatic nail guns without the use of eye and face protection. The employer did not ensure that employees were protected with eye and face protection, thus exposing employees to an eye injury, on or about 7/2/19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest:  a) Exterior Front: Employees engaged in roofing work were exposed to a fall hazard of approximately 16 feet above the lower surface, without the use of a personal fall arrest system. The employer did not ensure that employees performing roofing work were provided with a personal fall arrest system, or other means of fall protection, thus exposing employees to a fall hazard, on or about 7/2/19.
