29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Employees were exposed to electrical hazards. A periodic inspection of the lock out tag out program had not been performed annually. It had been a year and a half since the last periodic inspection of the lock out tag out program had been conducted.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: Employees were exposed to fire hazards. Employees in the Disassembly Area were using a welding torch cart that contained an Amerex portable fire extinguisher that was overdue for it's annual maintenance check. The fire extinguisher had last been inspected in July 2017.
