On the east wall, the employer did not ensure the blade of the evolution cutting machine guard lowered when cutting.  The employer had the guard bolted in the up position exposing employees to amputation hazards.

29 CFR 1910.213(i)(1): Nonworking portion(s) of the blade of the bandsaw were not enclosed or guarded.   On the east side of the production area, employees operated a metal cutting band saw and the unused portion of the blade was not guarded exposing employees to amputation hazards.  Approximately 12 inches of the blade was unguarded.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).    On the south side of the production area, the employees used a floor fan and the power cord outer sheathing was split exposing internal wiring.  The employees were exposed to electrical shock hazards.

29 CFR 1910.305(b)(3)(ii): 29 CFR 1910.305(b)(3)(ii): Boxes shall be closed by suitable covers securely fastened in place, boxes were not closed as required.    On the west side of the production area, the employer did not ensure the electrical panel box used to energize machinery and lights were effectively closed during operations.  The employees were exposed to electrical shock hazards and inadvertent energization of equipment.

On the west side of the production area, the employer allowed employees to turn fluorescent lights on and off from the electrical panel box and circuits that were not listed and marked "SWD".  The employees were exposed to electrical shock hazards.
