29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    (a) Roof above Classroom Wing - An employee installed TPO roofing at the corners along the north side of the roof.  This employee took down part of the warning line system along the north side of the roof, worked outside of the warning line system that remained up (including working right at the roof edge) wore a harness with a lanyard but did not tie off, and did not use any other fall protection system, exposing the employee to falling to the ground below, on or about July 3, 2019.    No abatement certification or documentation required.  Corrected during this inspection.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:    (a) Roof above Classroom Wing - An employee installed TPO roofing and used personal fall arrest, which included anchoring to a fall arrest attachment ring on an AES Raptor Manufacturing/Leading Edge Safety Raptor TRIREX mobile fall protection system anchorage unit.  This mobile fall arrest system anchorage unit did not comply with the requirements of the standard because the employer did not set up and use the unit in accordance with the manufacturer's instructions.  Specifically, the employee who anchored to the unit had the unit set up perpendicular to rather than parallel to the roof edge causing the employee to work 90 degrees rather than within 45 degrees or less of the fall arrest attachment rings, which could result in the engagement arm assembly not functioning in the event of a fall, exposing the employee to falling to the ground below, on or about July 3, 2019.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
