29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI).     The employer is to make this determination in accordance with either paragraph (d)(2), the scheduled monitoring option, or paragraph (d)(3), the performance-oriented option.     a. On or about July 3, 2019, in the welding area, the welder was required to weld stainless steel for the finished vehicle. The employees were exposed to chromium (VI) at a time weighted average (TWA) of 0.0008 mg/m3.  The employer did not perform initial air monitoring to determine the 8-hour TWA exposure for each employee exposed to chromium (VI). The limit was established to prevent nasal septum ulceration and perforation, dermatitis, asthma and lung cancer.

29 CFR 1910.1026(l)(1)(iii): The employer had not included chromium (VI) in the hazard communication program, established to comply with the Hazard Communication Standard, 29 CFR 1910.1200, ensured that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trained in accordance with the requirements of the Hazard Communication Standard and 29 CFR 1910.1026(l)(2), including the contents of the Hexavalent Chromium Standard, the purpose and a description of the medical surveillance program, and made copies of this standard available to all affected employees:    a. On or about July 3, 2019, in the welding area, an employee required to weld on stainless steel was exposed to chromium (VI) at 0.0008 mg/m3. The employer did not train the employees on the hazards associated with chromium (VI), the chromium (VI) standard, the medical surveillance paragraph and availability of the standard.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:       a. On or about July 3, 2019, in Building 2, the employee spray painting parts wore a half mask respirator. The employer did not ensure the respirator program was implemented adequately. The painter was not trained, fit tested or medically evaluated.      The Scaletta Moloney Armoring Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1), which was contained in OSHA inspection number 1226939, citation number 1, item number 2 and was affirmed as a final order on September 5, 2017, with respect to a workplace located at 6755 South Belt Circle Drive, Bedford Park, IL.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a. On or about July 3, 2019, in the work area, the temporary employees were required to perform tasks that created exposures to chemicals. The employer did not ensure that the employees were trained in the hazards associated with chemicals such as, but not limited to, toluene and welding fumes containing chemicals such as iron oxide.    The Scaletta Moloney Armoring Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 1226939, citation number 1, item number 5  and was affirmed as a final order on September 5, 2017, with respect to a workplace located at 6755 South Belt Circle Drive, Bedford Park, IL.
