(a) Site: On or about June 26, 2019, the employer exposed employees to fall hazards of 10.5ft to the ground in that, they were throwing debris from the roof without a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Site: On or about June 26, 2019, the employer exposed employees to fall hazards in that, fall protection training was not provided when employees were throwing debris and ductwork from the roof at a height of approximately 10.5 feet.

29 CFR 1926.1052(c)(9): Handrails for stairways did not provide adequate handholds for employees grasping them to avoid falling: (a) Site: On or about June 26, 2019, the employer exposed employees to fall hazards in that, handrails for stairways were not provided to them while using the stairways. The fall distance was approximately 36 inches to the floor.
