OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with the potential release from process piping such as, but not limited to chemical contact and chemical inhalation while working at or near process piping and their associated piping systems:  (a) On or about June 21, 2019, pressure relief devices installed on the discharge of positive displacement pumps in the development stage continuous flow reactor equipment bay arranged to synthesize 3,9-Diethylidene-2,4,8,10-tretraoxaspiro[5.5]undecane (DEtOSU) were not adequately set to provide relief and prevent the release of process streams to the immediate work environment in the case of a blocked outlet. This exposed employees to chemical contact hazards and inhalation hazards while performing work.
