29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  Site: At the construction site (Lots 1 and 2) Cain's Way (a.k.a. Cain's Crossing) Southborough, MA 01772 on 7/2/2019, and at times earlier, employees engaged in framing were working at a height greater than 6 feet and were exposed to wall holes which were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employees provided with  alternative fall protection measures.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  Site: At the construction site (Lots 1 and 2) Cain's Way (a.k.a. Cain's Crossing) Southborough, MA 01772 on 7/3/2019, and at times earlier, six employees worked on the second floor of the project and were exposed to fall hazards from unguarded wall holes and did not have fall protection training to recognize the hazards of falling nor the procedures to be followed in order to minimize these hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a)  Site: At the construction site (Lots 1 and 2) Cain's Way (a.k.a. Cain's Crossing) Southborough, MA 01772 on 7/3/2019, and at times earlier,  six employees worked on the second floor of the project and were exposed to fall hazards from open sided floors and unguarded floor holes and the employer had not verified compliance of this section by preparing a written certification record in accordance with this subpart for all employees exposed to fall hazards.
