On or about May 14, 2019, employees utilized a portable ladder to access the roof of a residential home located at 1271 Broadway Avenue, Bedford, OH 44146, and the employer did not ensure that it was utilized at a the proper angle in that the horizontal distance was 7 feet while the working length of the ladder was 12.45 feet.
