29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  23 Wilson Drive, Site #4004, Morristown, NJ.  Employee involved with exterior wall masonry work was exposed to struck-by injuries from falling masonry stones as no hard hat was being worn.                                                                              Violation observed on or about 07/01/2019.

29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   a)  23 Wilson Drive, Site #4004, Morristown, NJ.  Employees involved with exterior wall masonry work were exposed to falls of 21 feet to the ground below as the working level of the scaffold was not fully planked having only 4 planks forming the entire platform where at least 2 more planks were needed.                                  Violation observed on or about 07/01/2019.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   a)  23 Wilson Drive, Site #4004, Morristown, NJ.
