(a) Site: On or about January 21, 2019, the employer exposed employees to struck-by hazards in that they failed to conduct frequent and regular inspections of the job site to discover hazards while subcontractors were lifting precast concrete headwalls from a tractor trailer using a Komatsu Excavator.
