29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    On or about August 16, 2019, employees were removing TT Hydrate Intermediate, a corrosive substance, from super sack bulk bags using a scoop, for placement into kiln trays in the custom fire department, kilns #1, #3, #5, #6, while wearing short sleeve shirts and shorts.    Employees were wearing short sleeve shirts and shorts, exposing their skin to the corrosive substance.  The employer did not assess the workplace to determine if hazards are present, which necessitate the use of proper personal protective equipment (PPE).     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a. On or about July 3, 2019, employees were removing TT Hydrate Intermediate, a corrosive substance, from super sack bulk bags using a scoop for placement into kiln trays in the custom fire department roller hearth kilns #1 and #3.    There was no suitable eyewash station or safety shower  in the vicinity of the finishing tanks to quickly drench the eyes or skin of the employees in the event of exposure to corrosive materials in both rooms of the department.  The eyewash station at the time was not up to date and was not kept up properly.  Its contents were expired and not monitored.      b.  On or about August 16, 2019, employees were removing TT Hydrate Intermediate, a corrosive substance, from super sack bulk bags using a scoop for placement into kiln trays in the custom fire department roller hearth kilns #5 and #6.       There was no eyewash station or safety shower in the vicinity of the finishing tanks to quickly drench the eyes or skin of the employees in the event of exposure to corrosive substances in both rooms of the department.  There was no eyewash station accessible and in close proximity to the employees.  The employees needed to go to a different room and turn corners in order to access an eyewash station.        Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    West, Ipsen Industries Roller Hearth Kilns #1, #3, #5, and #6: On or about August 19, 2019, employees were exposed to corrosive materials i.e. TT Hydrate Intermediate, during their task of removing TT Hydrate Intermediate, from super sack bulk bags using a scoop, for placement into kiln trays in the custom fire department roller hearth kilns #5 and #1.       Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19 (d).

29 CFR 1910.134(k)(6): 29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, to employees who wear respirators when such use was not required by the employer.    On or about July 3, 2019, employees in the custom fire department in roller hearth kilns #1, #3, #5, and #6, were exposed to respiratory stressors associated with being allowed to voluntarily wear 3M 8210 filtering face pieces (dust masks) while transferring TT Hydrate intermediate from sacks into the kiln trays.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
