Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.    a)  Salinas Construction and Repairs LLC on or about July 5, 2019, at the address of 3242 Executive Pkway Toledo, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated location of 20 ft. without the benefit of fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  a)  Salinas Construction and Repairs LLC on or about July 5, 2019, at the address of 3242 Executive Pkway Toledo, OH: The employer did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. The employees were exposed to a potential 20 ft. to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.  a)  Salinas Construction and Repairs LLC on or about July 5, 2019, at the address of 3242 Executive Pkway Toledo, OH: The employer did not ensure the side rails of the 30 feet aluminum extension ladder, extended at least three feet above the landing surface. Two employees were exposed to falls of up to 20 feet while using the ladder to access the west side of the roof.
