29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about July 5, 2019, employees were exposed to potential shock and burn hazards when an outlet box located next to the furnace was not provided with an cover.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure. On or about July 5, 2019, employees were exposed to shock and burns hazards when: a) a Dayton model 5EAK1 mobile air conditioner was plugged into an extension cord that was then run to a power strip which was also providing power several computer components. b) a Sharp Carousel II microwave was plugged into a power strip instead of a wall outlet.
