29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a)   Ace Roofing & Construction on or about on July 5, 2019, at the address of 2330 Drummond Rd Toledo, OH: The employer failed to ensure employees were protected from a fall while working from a ladder jack scaffold.   Employees were exposed to a near 24' fall to the level below during roofing operations of a residential structure. Two employees were exposed to a near 24' fall to the ground below while performing roofing work on the south side of the roof. No form of fall protection was in place.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.  a)   Ace Roofing & Construction on or about on July 5, 2019, at the address of 2330 Drummond Rd Toledo, OH: The employer did not ensure the side rails of 14 ft., 24ft. and 30 ft. fiberglass extension ladders used onsite were extended at least three feet above the landing surface. Three employees were exposed to a near 12 ft. fall to the level below while using the ladder to access the west side of the 12 pitch roof.
