29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) Jobsite, Somerset Historical Center, 10649 Somerset Pike, Somerset, PA:  On or about June 27, 2019 employees who were spraying sealer on the cedar shingles on a 8/12 pitch roof of a covered bridge were not protected from falling between 10-22 feet by guardrails, safety nets or personal fall arrest systems.  An employee fell from the roof and sustained a broken right arm and compression fracture in their back.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    (a) Facility, 12 Western Avenue, Pittsburgh, PA:  On or about July 2, 2019 or after the employer did not have any written training certification records to show that employees had received fall protection training as required by paragraph (a) of this section.
