29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    a) Front yard - On or about July 4, 2019 and at times prior thereto, employees were removing a number of trees along energized powerlines and had not been trained in overhead electrical hazards.    On July 4, 2019 an employee was electrocuted while performing tree removal operations.

29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained:   a) 118 Pike St. - On or about July 4, 2019 and at time prior thereto, sub-contractors employee was operating a truck mounted crane, within 10' of power lines, lowering tree sections onto the ground for employees to trim.   On July 4, 2019 an employee was electrocuted while performing tree removal operations.

29 CFR 1910.333(c)(3)(iii)(B): Employees standing on the ground may not contact the vehicle or mechanical equipment or any of its attachments, unless:  (1)  The employee is using protective equipment rated for the voltage; or (2)  The equipment is located so that no uninsulated part of its structure (that portion of the structure that provides a conductive path to employees on the ground) can come closer to the line than permitted in paragraph (c)(3)(iii) of this section.  a) 118 Pike St. - On or about July 4, 2019 and at times prior thereto, the employer did not ensure employee(s) on the ground did not contact the vehicle, mechanical equipment, or any attachments without protective measures as required.  Employees were handling the rigging and crane hoist line from the ground while it was within 10' of overhead power lines.   On July 4, 2019 an employee was electrocuted while performing tree removal operations.
