29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  181 West Street, Ware, MA: On or about July 8, 2019 employees were observed engaged in roofing activities on a residential structure which was more than 20 feet above the next lower level and were not protected from falling by either guardrail systems, safety net systems or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  181 West Street, Ware, MA: On or about July 8, 2019 employees were observed engaged in roofing activities and exposed to fall hazards and had not been provided training in the recognition of fall hazards and means and methods to minimize the risk.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  181 West Street, Ware, MA: On or about July 8, 2019 the employer had not verified compliance with this section by preparing a written record (certification) for training to include the date of training, name or other identity of person(s), signature of person(s) trained and signature of the owner or the owner's designated person.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
