The employees were unprotected from in between the joist of the new build residential home. The employees were exposed to a fall distance of 9 feet 11 inches to the concrete ground level.

Located at 240 Honeywood Drive N, Galena, Ohio: On or about July 08, 2019, the employer failed to ensure that two employees who worked on the second level of a new build residential home were protected from falling off the second floor level. The employees were exposed to a fall hazard of 9 feet 11 inches without any means of fall protection.  Within the past five years, the employer had been cited for the same hazards in four different inspections.  Alejandro Vasquez Gallegos, was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1324337, citation number 2, item number 1 and was affirmed as a final order on July 24, 2018, with respect to a workplace located at 4351 McAlister Park Drive, Westerville, Ohio.
