29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:         (a)            Brown Note Productions, Inc. at 471 E. 124th Ave., Thornton, CO 80241:  On or before July 8, 2019, the employer was utilizing a man basket forklift attachment on the Big Joe power drive straddle truck (serial # 32420044). The manufacturers approval to use the attachment with the lift was not obtained. This condition exposed employees to hazards associated with possible tipping and overloading of the lift.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other that factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:         (a)            Brown Note Productions, Inc. at 471 E. 124th Ave., Thornton, CO 80241:  On or before July 8, 2019, the employer was utilizing a man basket forklift attachment on the Big Joe power drive straddle truck (serial # 32420044). The Big Joe straddle trucks data and capacity plates were not changed to reflect the attachment. This condition exposed employees to hazards associated with possible tipping and overloading of the lift.
