29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a)  At 7371 Blackmon Rd., Chattahoochee Valley Dental, Columbus, Georgia:  On or about July 9, 2019 and at times prior; the employer allowed his employees to perform framing activities without fall protection exposing his employees to a fall hazard of up to 11.7 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At 7371 Blackmon Rd., Chattahoochee Valley Dental, Columbus, Georgia:  On or about July 9, 2019 and at times prior; the employer allowed his employees to use and extension ladder to gain access to an upper level of the building and the rails of the ladder did not extend at least three feet above the landing to provide safe access exposing his employees to a fall hazard of 11.7 feet to the ground below.
