29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Warehouse left side overhead storage area: Employees working near an unprotected edge were exposed to a fall hazard to the concrete floor below.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Warehouse: access to a fire extinguisher was blocked by a mop, bucket, and metal panels preventing immediate access. Employees were exposed to fire hazards.
