The employee was working from a height of 12 feet while taping sprinkler heads in preparation for paint.

A) Absolute Multi Services LLC on or about July 9, 2019 at the address of 500 N Warpole Street, Upper Sandusky, OH: The employer did not ensure one employee working from a Genie Z-34/22 Aerial lift was trained in the recognition of hazards associated with this type of mobile scaffold. Fall hazards of no personal fall arrest system harness, went unrecognized while the employee performed demolition, remodeling, and gut rehab operations from the mobile scaffold.
