29 CFR 1910.119(l)(1): The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process (a) Location: heat exchanger number(s) 6/9 On or about July 11, 2019 and at times prior thereto employees were exposed to an uncontrolled release of benzene, explosions, and fire, because the employer failed to implement a management of change when a flange was installed inside of heat exchanger number(s) 6/9.

29 CFR 1910.1028(i)(4)(iii): For employees exposed to benzene in an emergency situation and whose urinary phenol test performed on the sample results were equal to or greater than 75 milligrams phenol per liter of urine, the employer did not provide the employee with a complete blood count including an erythrocyte count, leukocyte count with differential and thrombocyte count and monthly intervals for a duration of three months following the emergency exposure: (a) For the employees identified with a Phenol level greater than 75 milligrams phenol per liter of urine on or about June 17, 2019.
